For Outpatient SUD services, it is preferable that clients be at least of adolescence age; however, decisions will be determined on an individualized basis.

For the Partial Hospitalization “New Leaf” Program, women must be 18 years of age or older.

For Mental Health Services, decisions are determined on an individual basis, and depend upon the presenting problem.

What medications do you prescribe?

For MAT- ASCEND can, and does, utilize all buprenorphine containing medications approved by the FDA for the treatment of Opioid Use Disorder.  The answer to the question of which medication is most appropriate for an individual client is determined by the prescriber and that individual client.  In general, we utilize Suboxone, Subutex (for pregnant clients only), and Zubsolv.  In addition, we work closely with the Health Department to link clients wanting to utilize the Vivitrol injection.  We do NOT prescribe methadone.

ASCEND provides comprehensive Medication Management for Mental Health Conditions.

When will medication be prescribed?

Our goal is for a client to meet with their provider/prescriber the same day their assessment is completed, and this is typically the case.  Of course, the initial urine drug test and necessary bloodwork must be completed first.  One of our nurses coordinates with the client and provider/prescriber to set up the initial appointment.

Will you work around my work schedule?

Yes, and it is based on proof of employment (i.e. paystub), your Individualized Treatment Plan (created by you and your Primary Counselor), and the Level of Care which is determined most appropriate for you.  We will try to accommodate your work schedule to the best of our ability.  However, if being prescribed a MAT medication, you will be required to dose daily with one of our nurses for at least the first 2 weeks of services (this is for all clients regardless of previous treatment, and is at the discretion of the nurse and prescriber).
